About the Role
MD Natural Corp is a leading provider of CCM (Chronic Care Management), RPM (Remote Patient Monitoring), and BHI (Behavioral Health Integration) services. We are seeking dedicated, bilingual Certified Medical Assistants to join our team as Care Coordinators. In this role, you will act as a vital link between patients and their medical providers, ensuring they receive consistent, high-quality care from the comfort of their homes.
Key Responsibilities
• Conducting telephonic outreach to patients for CCM, RPM, and BHI services.
• Coordinating care plans and monitoring patient health data.
• Documenting patient interactions accurately in compliance with healthcare regulations.
• Collaborating with medical offices to improve patient outcomes.
Required Qualifications
• Certification: Must be a Certified Medical Assistant (CMA).
• Languages: Native or professional fluency in both Spanish and English is required.
• Experience: Previous experience in care coordination or chronic care management is preferred but not required.
• Skills: Strong communication skills, empathy, and basic computer proficiency.

• Remote: Work from the comfort of your home.
• Hours: Both Full-Time and Part-Time positions are available.
Compensation & Benefits
• Office-Based Pay: $20.00 per hour.
• Remote/Home-Based Pay: $11.00 per case (Average output is 2–3 cases per hour, totaling $22–$33/hour).
• Bonuses: Significant annual performance-based bonuses.
